Our SureStep™ Fall Prevention Program

Preventing the leading cause of injury in older adults.

The consequences of a fall can be devastating to your independence, as one misstep is all it takes to initiate a chain of challenging physical setbacks. Also, physical, mental and emotional health are all interconnected; when one is affected, it has a ripple effect on other areas of your well-being.

At HomeWell, we’ve seen how falls can not only cause injury and ongoing mobility concerns but also lead to depression, fear, and social isolation. Those at risk may withdraw from doing the things they love or performing daily tasks. If you or a loved one are concerned about falling and ready to regain your confidence in navigating everyday life, SureStep Fall Prevention is for you.

Purposefully designed to protect older adults from falls, this program starts with an in-home assessment by your local HomeWell’s expert Care Manager. They will find where and why you’re most at risk of falling and help you address those risks holistically through activity, engagement and more. With a comprehensive, highly personalized approach to care, SureStep can help you put your best foot forward toward achieving a happier, healthier life at home.

Did you know?

  • Falls are the leading cause of serious injury for older adults.1
  • 3 million older adults are treated in emergency departments for fall injuries every year.2
  • One out of five falls causes a serious injury such as broken bones or a head injury.3

How does SureStep protect you from falls?

The causes of falls are complex. HomeWell can help you understand and address them all.

Physical inactivity

Sitting for long periods can result in muscle weakness and loss of balance. HomeWell caregivers can help your loved one do more physical activity in safety.

Medications

Medications and their side-effects can increase fall risk, especially with multiple prescriptions. Your loved one’s care team can assess whether your loved one’s medication increases their risk of falling.

Haazards in the home

Your Care Manager can evaluate your loved one’s home for safety issues like poor lighting and trip hazards and recommend modifications for better safety and access.

Low vision

Having impaired vision doubles an older adult’s risk of a fall.5 HomeWell works with clients to understand and address the unique fall risks that come with low vision.

Prio fall history

An older adult’s first fall doubles their chances of falling again.6 It can also cause a fear of future falls, which may cause decreased activity and a sense of withdrawal.

Social isolation

Inactivity and social isolation lead to loneliness, depression and anxiety, which increase the risk of falling. Your caregivers can help your loved one do more of the activities they enjoy and even join in.

Companion Care

Chronic conditions often reduce strength, balance, and mobility. Our agencies’ caregivers are trained to work with clients with multiple conditions that present a heightened fall risk.
