What is the GUIDE Model? 

Launched by the Centers for Medicare & Medicaid Services (CMS), the Guiding an Improved Dementia Experience (GUIDE) Model is a nationwide program designed to improve the quality of life for people with dementia and reduce strain on their unpaid family caregivers. 

GUIDE provides coordinated care through: 

  • A dedicated Care Navigator for families 
  • 24/7 support lines for questions or concerns 
  • Access to caregiver education, training, and counseling 
  • Connections to community-based services like transportation and meals 
  • Respite care services for qualifying families, offering temporary relief
