GUIDE Dementia Care Support

Through our national partnership with PocketRN, HomeWell agencies nationwide can help families connect to the Centers for Medicare & Medicaid Services’ (CMS) Guiding an Improved Dementia Experience (GUIDE) Model, a groundbreaking government-backed initiative that provides comprehensive dementia care and family caregiver support. PocketRN’s goal is to help eligible families access the services, resources, and relief needed to keep their loved one safe and supported at home. For those who qualify, this can include up to 72 hours of Medicare-covered home care services annually—with no out-of-pocket costs. 

This comprehensive support can help: 

• Reduce hospitalizations and ER visits 
• Delay or prevent nursing home placement 
• Improve management of behavioral symptoms 
• Support physical, emotional, and mental health for both clients and caregivers 
• Address health-related social needs through community referrals 
• Provide financial relief through Medicare-covered services 

Launched by the Centers for Medicare & Medicaid Services (CMS), the Guiding an Improved Dementia Experience (GUIDE) Model is a nationwide program designed to improve the quality of life for people with dementia and reduce strain on their unpaid family caregivers. 

GUIDE provides coordinated care through: 

• A dedicated Care Navigator for families 
• 24/7 support lines for questions or concerns 
• Access to caregiver education, training, and counseling 
• Connections to community-based services like transportation and meals 
• Respite care services for qualifying families, offering temporary relief
