Every HomeWell client receives an expert Care Manager to coordinate their care at no additional cost. You can rest assured knowing your individual care plan is tailored to your or your loved one’s specific needs to provide a more meaningful, personalized experience. You can expect our Care Managers to help with:
- Providing ongoing care assessment and monitoring
- Fostering a holistic health approach to help ensure a higher quality of life
- Liaising between clients, families and service providers

The Commission on Accreditation for Home Care
To further the impact of HomeWell’s mission and our role within the continuum of care, we are honored to serve on the Board of Trustees for the Commission on Accreditation for Home Care (CAHC). The CAHC has been setting the standards for quality home care services since 1986, bringing in-depth knowledge of state legislation, rules and regulations and sharing ongoing collaborative relationships with state home care governance authorities.
