Historically, most diagnoses occur in young children, typically between the ages of five and six. 1 However, there is still quite a lot that experts have yet to understand about autism.

In a 2019 Harvard Health study of more than 1,200 toddlers revealed that about 2% initially suspected of having autism were later found to develop normally. Conversely, 24% of toddlers initially thought not to have autism were diagnosed later on.2

While the signs can be difficult to identify at first, it’s clear that when a diagnosis is made, it tends to stick. And the older a child gets, the greater the certainty of the diagnosis becomes. The same Harvard study showed that diagnostic confidence grew from 50% at 12-13 months to over 80% at 16 months of age.2

As experts’ understanding of ASD continues to improve, awareness of its presence in older adults is increasing rapidly. When today’s older generations were children, research was less advanced, so many individuals likely lived without diagnoses, potentially left wondering or even masking their symptoms. In response to today’s improvements in research, many older adults are now seeking or receiving an autism diagnosis for the very first time. Today, an estimated 1 in 100 adults may be on the autism spectrum.3

#DidYouKnow

According to a 2026 study by the National Autism Data Center, the following is true about older adults with autism:

At least 3 out of 4 middle-aged and older autistic adults are 40-64 years old.

About one-third of aging autistic people are female.

Nearly 80% of aging adults with autism are white.

More than 80% of aging autistic people live in urban areas.4

What to know about autism and aging

As people with autism grow older, their needs and experiences can evolve in ways that are not always widely understood. While autism is often discussed in the context of childhood, many adults on the spectrum face distinct challenges as they age. Understanding these common signs, potential health risks, and unique day-to-day difficulties that may arise can help families and care professionals provide more informed and supportive care.

Signs of autism in adults

Symptoms of autism appear differently in seniors compared to young people. In children who are rapidly growing and developing, signs tend to manifest more clearly as developmental delays. Older adults who have not yet been diagnosed, however, may have developed coping mechanisms over the years to hide their autistic traits, making it more difficult to definitively diagnose.

If you are unsure whether an aging loved one may be on the spectrum, a few common signs of autism in adults to look out for can include:

They struggle with social interactions

They have difficulty maintaining relationships

They have rigid routines and strongly resist change

They have a hard time interpreting nonverbal communication5

Health risks for adults with autism

Adults over the age of 65 who have ASD are more likely to experience physical challenges, such as epilepsy and gastrointestinal issues, and mental challenges, such as schizophrenia and personality disorders, than those without it.6 Some studies suggest that older adults with autism may have a higher likelihood of experiencing certain age-related conditions, including:

Cancer

Heart disease

Arthritis

Parkinson’s disease

Dementia and Alzheimer’s disease6

Unique challenges faced by adults with autism

As loved ones get older, symptoms of autism can become more pronounced. Depending on where they fall on the spectrum, these challenges can have lasting effects on many areas of their life. Social and communication struggles, two very common difficulties for adults with autism, can cause problems in the workplace, affect personal relationships, and have lasting mental health implications.

Communication difficulties

Older adults with autism may experience communication challenges that affect how they interpret language, express their needs, or engage in social interactions. They may also have difficulty reading social cues, understanding sarcasm and humor, or adapting to the back-and-forth flow of conversation, which can sometimes lead to misunderstandings or feelings of isolation if others aren’t aware of their communication style.

Sensory sensitivities

Sensitivities to loud noises, bright lights, or crowded spaces can all affect how a person responds to their environment. Everyday stimuli that many individuals may not even notice can feel overwhelming and uncomfortable to those with autism.

Social isolation

Sadly, social isolation is common among older adults, and autism can dramatically exacerbate it. Feelings of being misunderstood, communication challenges, and sensory sensitivities can make it difficult to form close relationships and build strong social circles. Too often, this leads to loneliness and reduced engagement with their surrounding community.

Cognitive changes

Aging is regularly associated with changes in cognitive function, from occasional forgetfulness to complex conditions like Alzheimer’s. When autism is also at play, those challenges can feel even more daunting. Many seniors with autism experience cognitive changes that are separate from those of typical aging, including difficulties with executive functioning, memory, and processing speed.5

Caring for an aging loved one with autism

Supporting an aging loved one with autism often requires thoughtful adjustments that honor their unique needs, preferences, and communication style. As they get older, these challenges can become even more noticeable and difficult to navigate — for them and their loved ones. By taking a compassionate and informed approach, families and caregivers can create supportive environments and help ensure their loved one continues to feel safe, understood, and valued.

Tailored activities and peer networks: Meaningful activities can help older adults with autism stay engaged and reduce their stress and frustration.
